Penis and glans augmentation with hyaluronic acid: volume and harmony

For those who don't want to hear about the operating room

“No. No way I’m having surgery.” That’s the answer I hear most often when I mention a surgical solution. And I understand it.

For many men, the operating room, anesthesia, recovery, or associated risks represent a significant barrier — even when the embarrassment is real and impacts self-confidence.

Penile hyaluronic acid: circumference increase without surgery, performed in the office .

Medical penoplasty exists precisely for these patients. The goal: to gain volume immediately, in a controlled and reversible manner .

Hyaluronic acid is a substance naturally present in the body, used for years in cosmetic medicine to restore volume. Its application here is to increase the circumference of the penis and glans without surgery.

But before going any further, one essential point: this technique has limitations that must be understood from the outset .

Hyaluronic acid: what it really is

A substance that your body produces naturally

Hyaluronic acid is a polysaccharide—a complex sugar—naturally present in connective tissues, skin, and joints. Its role in the body is to hydrate and maintain tissue volume . With age, our natural production decreases, which partly explains the loss of skin volume.

In cosmetic medicine, synthetic hyaluronic acid is used—cross-linked, meaning stabilized to last longer in the tissues. It is supplied in pre-filled, CE-certified, sterile syringes of controlled quality. This is the product I use in my practice for non-surgical penile augmentation.

Why hyaluronic acid is increasingly replacing lipofilling

Historically, the gold standard technique for increasing penile volume was lipofilling—the injection of autologous fat. It remains an excellent option, which I still offer to patients who choose it. However, hyaluronic acid has specific advantages that, in certain situations, make it a clearly superior alternative.

Main advantages:

  • No surgery
  • No general anesthesia
  • Immediate return home
  • Reversible procedure

What hyaluronic acid does — and what it doesn't do

Regarding the penis: volume, not length

I say this clearly because it is the source of most disappointments when information is not provided beforehand.

Main action: increase in circumference, not length .

If lengthening is your primary objective, we will discuss this during the consultation, and I may refer you to sectioning the suspensory ligament — with all the limitations that this procedure entails and that I have explained in a dedicated article.

On the glans: a rebalancing of proportions

Beyond the shaft, I also offer targeted hyaluronic acid injections of the glans. This procedure is particularly useful in two situations:

  • The glans is considered disproportionately small in relation to the shaft — an aesthetic rebalancing is therefore desired
  • After shaft augmentation with hyaluronic acid or lipofilling — the glans may appear smaller by contrast, and a slight augmentation restores the overall harmony of proportions

The volumes injected into the glans are intentionally kept moderate for safety reasons. The duration of the results is similar to that of the shaft: approximately 12 to 18 months .

What I’m looking for with these injections isn’t a spectacular or detectable result. It’s harmony . A penis with balanced proportions that gives the patient natural confidence—without his partner feeling like anything has changed.

What happens during an injection session? Step by step

Before the session: the consultation

No session takes place without a prior consultation. This is an absolute rule in my practice. During this consultation, I assess your anatomy, understand your expectations, and explain precisely what the technique can offer in your case. I inform you of the risks and obtain your informed consent.

If you have previously had hyaluronic acid injections elsewhere, I also assess this — to ensure that the previous product is absorbed before any further injections, or to decide if prior dissolution is necessary.

The day of the session

The procedure takes place entirely in the doctor’s office, without any hospitalization. Here’s what happens in practice:

  1. Application of an anesthetic cream — left in place 30 to 45 minutes before the injections to eliminate discomfort
  2. Targeted injections with flexible micro-cannulas — an average of about ten syringes during the first session, injected under the different layers of the penile wall
  3. Gently massage immediately afterwards — to ensure even distribution of the product and avoid irregularities
  4. Post-injection instructions given to the patient — to optimize distribution and results in the following days

Session duration: 20 to 45 minutes
Anesthesia: Local cream only — no general anesthesia
Hospitalization: None — returned home immediately after
Work stoppage: Generally none — return to work possible the next day

And after the session?

During the first 24 to 48 hours , slight swelling and some bruising are possible—this is normal and temporary. You will be given specific instructions for the days following the injection: avoid sexual intercourse for at least 48 hours , do not massage vigorously, and avoid intense heat (sauna, steam room) for one week.

Normal activities can usually be resumed the following day. This is one of the major advantages of this technique compared to surgery.

The results: figures, deadlines, and what happens over time

What you can expect after the first session

The result is visible immediately after the injections. The penis is wider as soon as you leave the office. But — and this is a crucial point — this initial result is not the final result .

Hyaluronic acid undergoes approximately 30% resorption in the first few months following injection. The result gradually stabilizes. This is why I systematically schedule a touch-up about one month after the initial session, if any irregularities appear or if the volume is deemed insufficient after stabilization.

Average gain: +2.5 cm in circumference after session + touch-up.
Resorption: ~ 30% in the first few months.
Duration: 12 to 18 months .

And in the long term?

Hyaluronic acid is gradually absorbed over a period of 12 to 18 months . After this time, the volume decreases. To maintain the result, touch-up sessions are necessary at this frequency.

For some patients, medical penoplasty is a progressive process over several years: each follow-up session can be an opportunity to slightly increase the volume, with tact and moderation, to obtain a final result more pronounced than what a single session would have allowed.

One point I want to clarify: length vs. circumference

The question everyone asks, and that I always answer honestly: does it also take longer?

No—not significantly . By becoming larger, the penis may appear slightly longer at rest. But this isn’t an actual lengthening of the corpora cavernosa—it’s an optical effect related to the increased mass. Expectations of lengthening with this technique will not be met, and I prefer to tell you that now.

Hyaluronic acid or lipofilling: how to choose?

This is often the question that comes after the initial explanation. Both techniques have the same goal—increasing volume—but they are suited to different profiles. Here’s an honest comparison to help you decide before your consultation.

care.
Hyaluronic acid lipofilling criterion
Type of medical procedure — office without surgical suite — operating room
Anesthesia: Local cream, general or regional
Session duration: 20–45 minutes or 60–90 minutes
Hospitalization: None. Outpatient or 24-hour
Recovery: 24–48 hours, 2–3 weeks
Visible results immediately, 8–10 weeks
Average gain : +2.5 cm in circumference , +2 to +3 cm in circumference
Durability 12–18 months (resorption ~ 30% ). Permanent if weight remains stable.
Reversible? Yes (hyaluronidase) No
Touch-up possible after 1 month. Possible after 3–6 months if insufficient.
Suitable if… Afraid of the blockage, wants quick results, wants durability, no repeated stress

My practical recommendation

If you’re still unsure what you want, start with hyaluronic acid. You’ll see how your body reacts, how you feel about the results, and whether you’re happy with the outcome. And if in 12-18 months you want something more permanent, lipofilling will still be an option.

If, on the other hand, you already know that you want something permanent, that the constraint of repeated sessions weighs on you in advance, and that you accept a recovery of a few weeks — lipofilling will probably be more suitable for your project.

The risks: what you need to know before making a decision

I never administer an injection without clearly explaining the possible side effects. This isn’t to scare you—it’s to allow you to give informed consent.

Risk / Side Effect Frequency What I am putting in place
Asymmetry or irregularities possible. Technique using micro-cannulas, post-injection modeling.
Subcutaneous nodules: Rare if technique is rigorous. Touch-up or dissolution with hyaluronidase.
Migration of rare cohesive product selected, precise sublayer injection
Swelling and bruising: Frequent (transient). Apply a cold compress, rest for 24 hours , and follow massage instructions.
Uneven absorption possible. Touch-up at 1 month scheduled according to the protocol.
Infection : Very rare. Medical office, strict aseptic technique.
Exceptional allergic reaction . Prior assessment, CE-certified products.

The antidote: the major advantage that few techniques offer

One of the things that allows me to approach the risks with more serenity with my patients is the existence of hyaluronidase — the antidote to hyaluronic acid.

If the result is disappointing, if an irregularity appears, or if the patient changes their mind, an injection of hyaluronidase into the treated area dissolves the product. It’s not instantaneous—dissolution takes a few days—but it’s a reversal option that neither lipofilling nor lengthening surgery can offer.

Hyaluronic acid is dissolvable (hyaluronidase) .

Who is this technique for? The right profiles

The indications I encounter during consultations

  • Fear of the operating room or general anesthesia — HA is performed entirely in the office
  • Need to quickly resume your professional and personal activities — recovery in 24-48 hours
  • Desire to test before a more definitive commitment such as lipofilling
  • Concern about volume or circumference has a real impact on self-confidence.
  • Rebalancing the proportions — shaft and glans — for overall harmony
  • Realistic expectations — girth gain accepted as the primary objective, no lengthening expected

Cases where I recommend a different approach

  • Expectation of significant lengthening — the section of the suspensory ligament will be more suitable (within its own limitations)
  • If you’re looking for a permanent result without the need for follow-up appointments, lipofilling is the more suitable option.
  • Previous product still present — wait for complete absorption or proceed with prior dissolution
  • Active infection in or around the area — absolute contraindication
  • Uncontrolled clotting disorder

What I love about this technique is that it provides real improvement to patients who otherwise wouldn’t do anything —because the idea of surgery completely paralyzed them. Sometimes, simply removing the barrier of the operating room is enough to change an intimate life.

FAQ: All your questions about medical penoplasty with hyaluronic acid

The price of a non-surgical penile enlargement session generally ranges from €1,000 to €2,500, depending on the amount of product used and the practitioner. At my practice, a precise quote is provided during the consultation, based on your anatomy and desired results. Maintenance sessions (approximately €1,000 to €1,500 ) are recommended every 12 to 18 months to maintain the results.

The procedure is performed under local anesthetic cream, applied 30 to 45 minutes before the injections. Discomfort during the session is generally very well tolerated. After the injection, slight swelling and local sensitivity are possible for 24 to 48 hours —easily treated with cold compresses and simple analgesics if necessary.

On average, an initial session followed by a touch-up one month later results in a gain of approximately 2.5 cm in circumference —which corresponds to what the majority of my patients are looking for. This gain varies depending on the initial anatomy, the injected volume, and individual absorption. I never guarantee a precise figure before the consultation.

No — not significantly. The effect is primarily on the circumference. A slight visual effect of lengthening at rest may be perceived due to the overall increase in volume, but this is not actual lengthening. If lengthening is your priority, we will discuss this during the consultation, and I will assess with you whether lengthening surgery is a suitable option for your case.

This is one of the great advantages of hyaluronic acid: it is dissolvable . An injection of hyaluronidase into the treated area allows the product to dissolve within a few days. The result is therefore not irreversible—making it a particularly reassuring option for patients who want to try it out before committing to something more permanent.

The results typically last 12 to 18 months before significant resorption occurs. Touch-ups at this frequency help maintain the achieved gain. Some patients choose to gradually increase the volume at each follow-up session, in a phased approach over several years.

In principle, yes — but this is something that is assessed in consultation, on a case-by-case basis. The two products can coexist in the tissues, but homogeneous distribution and a clear clinical indication must be ensured before combining the two techniques.

No. The injections are administered under the superficial layers of the penile wall, around the corpora cavernosa—without touching them. Penile sensitivity and erectile quality are not affected. This is something I systematically check during post-injection follow-up.

Yes, both procedures can be performed during the same session. Glans augmentation is particularly relevant when shaft volume is also increased, to maintain harmonious proportions. The volumes injected into the glans remain moderate—it’s a sensitive area that requires a precise and conservative technique.

Key takeaways

Hyaluronic acid is the most accessible solution for increasing circumference without surgery .

Result: measurable, progressive, and adaptable over time.

This is not a radical transformation — it is a controlled improvement .

In conclusion, you don’t have to live with an inferiority complex because of your fear of surgery. There is a serious medical alternative, performed by a specialist in a rigorous setting—and it can change your relationship with your body.

If you’re reading this article and recognize yourself in the profile of a man who fears the operating room but truly suffers from this complex, please schedule a consultation. Together, we’ll find the right approach for you.
