Why I consider lipofilling to be the gold standard technique for volume
When a patient comes into my office and tells me that he wants to increase the size of his penis, the first thing I ask him is what he is really looking for: an immediate and reversible result, or something lasting, natural, that lasts over time?
Depending on the objective, the approach is different. In most cases, when a patient seeks a lasting and natural result , their own fat becomes the best option.
Penile lipofilling—which I also call lipostructure —is a technique I’ve been practicing for many years and which continues to give me excellent results. Not because it’s perfect, but because it uses what you already have —your own tissue—to create something natural, long-lasting, and that integrates perfectly with your anatomy.
In this article, I explain everything: the principle, the steps of the intervention, what results can be expected, how the recovery takes place, and above all — the limitations that I always honestly tell you before deciding together.
Penile lipofilling: what exactly are we talking about?
A two-stage surgical procedure
Penile lipostructure is based on a simple but precise principle: fat is taken from your own body — abdomen, flanks, hips, thighs — and reinjected into the tissues of the penis to increase its circumference.
Because the fat comes from your own body, there is no risk of rejection . It’s an autograft—the material is biological, living, and once it has taken, it behaves exactly like the surrounding tissue. This is the major difference with hyaluronic acid.
The main advantage: a natural result, undetectable to the touch .
What does this change in practical terms?
Lipofilling affects the circumference of the penis, both at rest and erect—in different proportions. During erection, the increase in volume is real but more moderate than at rest, because the corpora cavernosa, by stiffening, “occupy” the space.
The glans itself is not altered by lipofilling. If a rebalancing of the glans’ volume is desired after the procedure, this can be achieved through hyaluronic acid injections at a later stage.
How does the procedure unfold? The four stages
Penile lipostructure is a procedure I perform in an accredited surgical facility, under general or regional anesthesia. The patient usually leaves the same day—it is an outpatient procedure in most cases.
- Fat is marked and harvested — from the abdomen, flanks, or thighs depending on your body shape — using gentle micro-liposuction with small, almost invisible incisions.
- Fat preparation — decantation and centrifugation to retain only the most viable fat cells and purify the product
- Micro-wire reinjection around the corpora cavernosa — a precise technique that guarantees homogeneous distribution and avoids irregularities
- Light dressing — monitoring in the recovery room, return home the same day in most cases
Procedure duration: 60 to 90 minutes
Anesthesia: General or regional
Hospitalization: Outpatient (same-day discharge) or 24 hours depending on the case
Structure: Accredited surgical clinic, Paris
Can lipofilling be combined with other procedures?
Yes — and it’s often relevant. During the same surgical procedure, I can also perform a section of the suspensory ligament if the patient also wishes to increase flaccid length. This way, both length and volume are addressed in a single procedure, under a single anesthetic.
Other complementary procedures are possible depending on the needs: frenulum plasty, circumcision, or glans augmentation with hyaluronic acid during a later consultation to rebalance the proportions.
The results: what you can realistically expect
The gain in circumference
Average gain: +2 to +3 cm of circumference at rest .
When erect: real but more moderate gain.
These figures are consistent with data published in the literature, particularly in Translational Andrology and Urology , and correspond to what I observe in my daily clinical practice.
What I always tell my patients is that the exact figure is impossible to guarantee before the operation. Every anatomy reacts differently.
The good news: if the result is insufficient after 3 to 6 months of stabilization, a second lipofilling session is perfectly possible . It’s included in the protocol—it’s not a failure, it’s a step in the process.
When will we see the final result?
This is one of the questions all my patients ask me—and the answer requires a little patience. In the first few weeks, the penis is swollen due to post-operative edema. This swelling can give the impression of a very good result… which partially disappears as the edema subsides.
Stabilized result: It takes 8 to 10 weeks for the swelling to completely subside and for the volume to truly stabilize. The final result—the one that will last—is only clearly visible after a minimum of 3 months , sometimes 6 months .
Possible resorption: up to 30% of the injected fat.
Does the result really last?
Yes—on one essential condition that I always explain: you must maintain a stable weight . The injected fat behaves exactly like the rest of your body’s adipose tissue. If you lose a lot of weight, the penis will decrease in size. If you gain weight, it will increase.
This is a physiological reality — not a limitation inherent to the technology.
Weight stability is not an insurmountable obstacle. It is an essential criterion in your decision. For the vast majority of my patients who maintain a stable weight, the results are long-lasting—and this is what fundamentally differentiates lipofilling from hyaluronic acid.
Special case: lipofilling, penile implant and volume restoration
I wanted to address a question that I receive more and more often, and which deserves a clear answer: can a man who has lost penile volume following treatment for prostate cancer regain that volume?
The answer is yes — but not solely through lipofilling . In these patients, the volume loss is linked to devascularization of the corpora cavernosa caused by prostatectomy or radiotherapy. The tissues have become depleted and retracted.
In these specific cases, it’s the insertion of a penile implant—using a specific technique I call “oversizing”—that acts as an internal dilator and allows for a gradual regain of volume. The new generations of implants available on the French market for the past few months even allow for further expansion, thanks to multidirectional growth in both length and width when inflated to their maximum capacity.
Lipofilling, in this context, can be considered as a complement or as a second step—but it is not the primary solution for these patients. The correct diagnosis, established during the consultation, will determine which approach is best suited to your situation.
Recovery after penile lipofilling
What you need to prepare for
| Period | What happens | What I recommend |
|---|---|---|
| Day 0 – Day 2 | Significant edema, possible discomfort, light dressing. | Rest, simple pain relievers, loose-fitting underwear. |
| Days 3-7: | Swelling gradually reduced, mobility restored. | Showers allowed, no baths. |
| Days 7-14: | Active healing, minimal residual pain. | Follow-up appointment, strict hygiene. |
| 3 weeks. | Return to work possible. | No intense sport, no cycling/motorcycling. |
| 6 weeks. | Resumption of sexual relations possible | . Gradual, depending on clinical progress. |
| 8-10 weeks: | Result stabilizes, edema almost completely resolved. | Follow-up consultation for final evaluation. |
| 3-6 months. | Final result clearly visible. | Follow-up session if deemed necessary. |
The postoperative effects of penile lipofilling are generally well tolerated. It is not a painful surgery in the strict sense — but significant swelling should be expected in the first few days, which can be surprising if one is not prepared for it.
Essential practical guidelines
- Loose-fitting underwear is recommended for the first two to three weeks — absolutely avoid tight briefs that compress
- Showers are permitted from day 2-3 . Baths and swimming are not recommended for 3 weeks.
- No intense sport, no cycling, motorcycling or horseback riding for a minimum of 4 to 6 weeks
- Avoid sun exposure of the donor area for 2 months (risk of pigmentation).
- Sedentary professional activity: resumption possible after 1 to 2 weeks
- Follow-up consultations at 7-10 days , 1 month and 3 months post-surgery — essential
One point I always insist on explaining: the initial edema
In the first few days after the procedure, the penis may appear very swollen. I mean very . This is normal—it’s post-operative swelling, not the final result. I emphasize this point because some patients worry in the opposite way: they think the result will disappear completely. That’s not the case either.
The truth lies somewhere in between. Part of that initial volume is edema, which will subside. Another part is fat that has accumulated—and that will remain. Patience is part of the process .
Lipofilling or hyaluronic acid: how to choose?
This is the most frequent question in consultations, and it deserves a structured answer. These two techniques are not rivals—they address different profiles and can even be complementary.
| criterion | (autologous lipostructure) | Hyaluronic acid |
|---|---|---|
| Product origin: | Your own fat (autologous — zero waste) | Biocompatible synthetic gel |
| Main result: | +2 to +3 cm in circumference. | Immediate increase in circumference. |
| durability | if weight remains stable: | 12–18 months (resorption ~ 30% ) |
| Reversible? | No | Yes (hyaluronidase) |
| General or regional | anesthesia | ,local cream |
| Intervention duration: | 60–90 min | / 20–45 min |
| Outpatient or 24-hour | hospitalization | :None (office) |
| Recovery | 2–3 weeks, | 24–48 hours |
| Texture to the touch: | Very natural (living tissue) | Natural (soft gel) |
| Main risks: | Partial resorption, weight fluctuations | , nodules, asymmetry, migration |
| Indicative price | €2,500–€5,000 € | 1,000–€2,500 |
My personal approach
For patients who come to see me for the first time and are hesitant, here’s what I usually tell them: if you’re not sure you want something permanent, start with hyaluronic acid. It’s reversible, quick, and it gives you a concrete idea of the result you want.
If you already have a clear idea of what you want, if you do not want to come back every 12-18 months for a touch-up session, and if you are ready for a surgical procedure with a recovery period of 2 to 3 weeks — then lipofilling is most likely the best option for you.
In some cases, the two techniques combine very well. Hyaluronic acid can refine the result, particularly around the glans.
Who is penile lipofilling for?
The right directions
- Aesthetic discomfort related to the size of the penis at rest or erect, with an impact on self-confidence
- Desire for lasting results without regular maintenance
- BMI compatible with sufficient fat harvesting (tissue needs to be harvested)
- Stable weight or desire to maintain it after the procedure
- Realistic expectations: natural and proportionate improvement, not radical transformation
- Functional erection or treated and balanced erectile dysfunction
Cases where I advise against it or direct it differently
- Very low BMI: not enough fat available to harvest a sufficient volume of quality
- Significant weight fluctuations are expected or are typical — the result will be unstable
- Expecting immediate results without downtime — hyaluronic acid will be more suitable
- Post-prostatectomy volume loss: lipofilling alone is not the primary solution — a penile implant with oversizing is more appropriate in this context
I often tell my patients: lipofilling isn’t for everyone—but when it’s right for you, the results are among the most satisfying I see in my practice . The key, as always, is to properly determine the indication.
The risks: what you need to know before deciding
I have never performed a procedure without explaining the risks to the patient. This is not a way to frighten them—it is a way to respect their ability to make an informed decision.
Specific risks of penile lipofilling
- Partial fat resorption : Between 20 and 30% of the injected fat may be reabsorbed in the first few weeks. This is a normal biological phenomenon and is taken into account in the planning.
- Irregularities and nodules : If the reinjection is not homogeneous, irregularities may appear. The micro-thread technique reduces this risk, but does not eliminate it completely.
- Asymmetry: rare but possible, may require retouching
- Weight-related variation: the result changes with your weight — this is not a complication but a factor to consider
- Hematoma or seroma at the biopsy site: rare, easily treated if detected early
- Infection: very rare in approved facilities with antibiotic prophylaxis
What remains unchanged
- Penile sensitivity is not affected — the nerves are not involved in the reinjection area
- The quality and rigidity of the erection are not altered — the corpora cavernosa are not affected.
- The angle of erection remains unchanged — unlike when the suspensory ligament is severed
These guarantees are important. In my experience, lipofilling is one of the penoplasty techniques with the least functional impact. This is one of its advantages.
FAQ: Your questions about penile lipofilling
Is the result of lipofilling truly permanent?
Permanent, yes— provided you maintain a stable weight . The injected fat is living tissue that reacts to weight fluctuations just like the rest of your adipose tissue. If you lose a lot of weight, the volume will decrease. If you gain weight, it will increase. For a man whose weight is stable over the long term, the result is indeed permanent.
How many centimeters can be gained in circumference?
In practice, the average gain is 2 to 3 centimeters in circumference at rest . During an erection, the gain is real but more moderate. I cannot guarantee a precise figure before the operation, as graft survival varies from person to person. What is certain is that a second session can be considered if the result is deemed insufficient after 3 to 6 months .
Can a lipofilling procedure be repeated if the result is not sufficient?
Absolutely. It’s even included in the protocol. If, after complete stabilization—that is, 3 to 6 months —the result seems insufficient, a second lipostructure session is perfectly feasible. You simply need to ensure that you still have enough fat available for harvesting.
How long after hyaluronic acid injections can lipofilling be performed?
It is best to wait for the product to be completely absorbed—between 6 and 9 months, depending on the type of hyaluronic acid used. Operating on tissues still saturated with hyaluronic acid can complicate the even distribution of fat. This timeframe is assessed during the consultation.
Does lipofilling affect sensitivity or erections?
No. The corpora cavernosa are not affected by the procedure—the reinjection takes place around them, not inside. Penile sensitivity and erectile quality are not altered. This is one of the significant advantages of this technique compared to lengthening surgery, which can change the angle of the erection.
How much does penile lipofilling cost?
The price generally ranges from €2,500 to €5,000 , depending on the complexity of the case and the surgical approach. If an additional procedure is required (ligament section, etc.), the overall cost will be adjusted accordingly. This procedure is not covered by French national health insurance. A personalized quote will be provided during the consultation.
Should I stop working after the operation?
For office or sedentary work: allow 1 to 2 weeks of rest. For physical work: 2 to 3 weeks . Sexual intercourse is not recommended for 4 to 6 weeks . Strenuous exercise, cycling, and similar activities should be avoided for at least 4 to 6 weeks .
Can lipofilling help to regain volume lost after prostate cancer?
In this specific case, lipofilling alone is generally not the primary solution. The volume loss associated with a prostatectomy or radiotherapy is caused by devascularization of the corpora cavernosa—the tissues have become depleted in depth. This is where the placement of a penile implant with an oversizing technique is more appropriate. Lipofilling can sometimes be considered as a complement. This is something we will discuss together during a consultation, based on your specific situation.
Key takeaways
In my practice, penile lipofilling is the volume augmentation technique I most often recommend when patients are seeking a lasting and natural result . Not because it’s without drawbacks—it requires a recovery period, patience to see the final result, and a commitment to weight stability. But because, when the indication is well-defined, the results are solid and satisfying in the long term.
This is not the technique for everyone. It is the technique for patients who have a long-term vision of their well-being, who accept a recovery period, and who want something that will not disappear in 12 months .
If you recognize yourself in this profile, the next step is a consultation — to assess precisely what is possible in your case.
Penile cosmetic surgery is not a decision to be made on a whim. It is a decision that must be carefully considered, guided, and tailored to the individual .
